This Advanced Nurse Practitioner role will involve working alongside our multi-disciplinary team which consists of a GP Partner, Advanced Nurse Practitioner Partner and Pharmacist Partner. They are all supported by Advanced Nurse Practitioners who lead on acute same day care, alongside the Home Visiting Team and First Contact Physio, Pharmacists, Practice Nurses and HCAs. We also have Social Prescribers who work as PCN staff dealing with the Mental Health of our patients.

The successful Advanced Nurse Practitioner will work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, telephone or video consultation.

Main duties of the job

  • The Advanced Nurse Practitioner (ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
  • The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
  • ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

Job description

Core principles:

  • To be courteous and polite to all people associated with the practice on the telephone, face to face or in written form.

  • To support and participate in initiatives to ensure a safe and healthy environment for all practice users. This includes following procedures to ensure the control of potential hazards to health and safety.

Primary Responsibilities:

  • The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
  • Develop, implement and embed health promotion and wellbeing programmes
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure SNOMED CT codes are used effectively
  • Maintain chronic disease registers
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess and refer patients presenting with mental health needs
  • Be an extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF, etc.), complying with local and regional guidance
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion where appropriate

Secondary responsibilities:

  • In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:
  • Act as the audit lead, effectively utilising the audit cycle
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the organisation
  • Opportunities to develop an area of specialist interest, taking the lead within the organisation
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)
  • Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Learning and development:

The effective use of training and development is fundamental to ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role. All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme.

Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately enable them to improve processes and service delivery.

Qualifications:

Essential

  • Registered with Professional body
  • Relevant Nursing/health degree
  • Experience working in General Practice/Primary Care setting

Experience:

Essential:

  • Experience of working to achieve standards within the Quality and Outcome Framework (QoF)
  • Working autonomously in an Advanced Practitioner role
